Your support team
You are in charge of who supports you and this will usually be more than one person. We carefully match your support team to your preferences and goals. You’ll be involved in selecting the people in your support team.
It is our goal that you have a positive, mutual and respectful relationship with your team of support workers.

Community Connections offers regular support that is planned and rostered to suit your needs.
This support will be provided using the funded hours allocated to you by the Needs Assessment Service Coordination (NASC).
